What is ERP?

Exposure and Response Prevention (ERP) is a specialized form of Cognitive Behavioral Therapy (CBT) designed to treat Obsessive-Compulsive Disorder (OCD) and other anxiety-related challenges.

The idea is simple, but powerful:

Instead of avoiding the things that cause anxiety or relying on rituals or compulsions to feel better, you gradually face your fears in a safe, supportive way. With practice, you’ll learn that you can tolerate anxiety – and over time, the fear loses its power.

ERP is considered the gold standard treatment for OCD because it helps people break free from the endless cycle of obsessions and compulsions.

  • Starting ERP can feel intimidating, that’s normal. The good news is, your therapist will guide you step by step. Here’s what the process looks like:

    • Collaborative planning — Together, you’ll identify the specific fears, triggers, or situations that cause anxiety. You’ll also discuss your current responses (rituals, avoidance, or compulsions).

    • Creating a hierarchy — You and your therapist will build a personalized “ladder” of fears, starting with manageable challenges and gradually moving toward bigger ones.

    • Exposure — You’ll intentionally face a feared thought, object, or situation in a structured way. Exposures are never forced—you set the pace.

    • Response prevention — Instead of performing a ritual or compulsion to relieve anxiety, you’ll practice resisting it. Over time, this teaches your brain that anxiety naturally fades without needing the compulsion.

    • Learning new skills — Along the way, you’ll develop tools to manage distress, challenge unhelpful thoughts, and cope with uncertainty.

  • When we avoid or rely on compulsions, anxiety only grows stronger. ERP interrupts this cycle by teaching you to face your fears and trust your ability to cope. Over time, your brain learns that you’re safe—even when uncertainty or discomfort is present.
